VETERANS and Volunteers SIGN-UP

We want to offer our services to all qualified veterans. We prioritize combat wounded veterans, but if we have space, we will take service injured veterans.

Please fill out this form and provide documents if available. The veteran or a family member or friend can assist the veteran in applying. We will contact you in the future and we look forward to meeting you.

If you are a Veteran or a civilian that would like to volunteer to help our program, please sign up and say you want to volunteer. Some of our Vets go on activities and volunteer. We need logistical help and well as activity volunteers, so please reach out.

Veterans- The DD214 is required, and a VA Award Letter are recommended for you to qualify for our program. Please provide a brief summary of your situation and any special needs. (Be sure to cover the social security numbers on all pages, and for your privacy, only send the SUMMARY PAGE of your award letter. We do not want to see your personal health specifics.)
